T-shirt Order Form

Below is the order form for T-Shirts, Hoodies and Jackets. Shirts are only available periodically. We will be placing an order on 12/11/23. All forms and money must be turned in by the 11th. Any forms turned in after that date will not be ordered. Please reach out to SBN Maus with any questions.

Parents & Students:

Terminology, practice videos, & student rank requirements can be found by logging into Kicksite and going to the Library feature. Currently, there are videos of Forms, Hand Techniques, Foot Techniques, & Children’s One Steps, we are working on adding videos of other requirements for each rank. WHITE, YELLOW, ORANGE, AND GREEN BELT STUDENTS WILL BE REQUIRED TO HAVE A MINIMUM OF 16 CLASSES PER TESTING CYCLE PRIOR TO TESTING AND RED, CHO DAN BO, AND BLACK BELT STUDENTS WILL BE REQUIRED TO HAVE A MINIMUM OF 32 CLASSES PER TESTING CYCLE PRIOR TO TESTING.
